How to create a breakdown Report?

1. From the CMMS dashboard click Work Order Corrective. See Figure below.

2. Check for date from and to and click Add button. Date from and to must be the range of the date the corrective maintenance schedule date, otherwise you cannot view the added entry. Then click Add button.

3. Now all the textboxes and buttons that need entries will be enabled. W.O. No.: the system will get the last available W.O. No. Fill up data especially W.O. No, Eq. ID, Sched. Date, and Date Reported. Getting equipment ID, please click the button and select an equipment you would like to create a breakdown.

4. Then click Save button.

5. Now a Work Order has been created. Highlight the Work Order and click Labor/Materials Needed tab.

6. If in case there is/are material/s needed click Add Material>> button. A vacant row will appear in the lower grid. Enter value in each column if needed.

7. After entering values in every column and if there is a tax for every material cost click the Tax checkbox to check.

8. Entering values in a cell and press enter or removing focus on the cell will save the data or you can click Save button. Next click CM Task tab.

9. Click Add Task button. A popup of Corrective Libraries will appear.

10. Highlight a corrective maintenance task and click SELECT button or you can double click mouse. Note, if you cannot find one you can just select one and edit it. If you want it to appear in the popup add it in the setup of Corrective Maintenance Libraries.

11. You can still edit the Task Description. Complete the entries.

12. Enter Remark if any abnormality only column.

13. Now click Image tab.

14. Click 1.Upload Image Before button.

15. Click Browse button.

16. Locate and select image and click Open button.

17. Then click Submit Picture button.

18. Again click 2.Upload Image Before button. Two images needed in the breakdown report. Image Before is needed in breakdown report. The Image After is for accomplishment report.

19. Click Browse button.

20. Locate and select image and click Open button. Then click Submit Picture button. Same procedure for Image 1.

21. Image 1 and Image 2 has been uploaded.

22. In the Print pulldown menu click WO-Breakdown Report.

22. Click Print Work Order button.

Note : Be sure to fill up in (Figure 3 : Corrective Work Order List) the Customer Complaint, Cause of Trouble and Recommendation.
